Want to do~ ~たい
First get the ~ます masu form of the verb you want to do. Then drop the ~ます masu and add ~たい tai.

Of course if you want to say "do you want to..." Just add ka

Do you want to eat cake?

食べる(たべる)〓 to eat
たべたい (Tabetai)〓want to eat
たべたいです (Tabetai desu)〓want to eat (polite form)
Take off the ending い and add くない or くありません, depending on the form you want.
たべたくない (Tabetakunai)〓don't want to eat
たべたくありません (Tabetaku arimasen)〓don't want to eat (polite form)
Its the same as making any -i adjective negative =]

”-れる” and ”-られる"
The potential form {can, is able to, etc} of the verb is treated as an ichidan verb. So you do the same as you would for and ichidan verb, that is place it in Mizenkei by removing the "る" and then add "-たい".
NOTE: ”-れる” is for Godan Verbs and ”-られる" for Ichidan verbs.
食べる - to eat
食べられる - to be able to eat [something]
食べられたい - to want to be able to eat [something]
Then you can add the past tense, negative past tense, negative tense, etc. etc. by doing what Mattan said earlier.
EXTRA ADDITIONAL INFO THAT MAY CONFUSE YOU: This "-れる" "-られる" construction is USUALLY only used for Ichidan verbs. Godan verbs use a different construction, but can also use this one as well by using the Mizenkei form of the Godan Verb. Also, the "-れる" "-られる" construction can be used as the Passive form of the verb.
食べる - to eat {active form}
食べられる - ...is being eaten {passive form}
ケーキを食べられる The cake is being eaten.
That sentence can also mean, "I'm able to eat the cake.", it all depends on context.
